Retreat Participant Release and Waiver

I, the undersigned, voluntarily choose to participate in a Warrior Moms Support Foundation retreat and related activities.

Assumption of Risk and Release of Liability
I understand that participation in retreat activities may involve certain risks. In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless Warrior Moms Support Foundation, Inc., its officers, directors, volunteers, sponsors, retreat hosts, and representatives from any and all claims, liabilities, damages, or causes of action arising from my participation, except those resulting from gross negligence or willful misconduct.

Medical Treatment Authorization
In the event of an emergency, I authorize Warrior Moms Support Foundation representatives to obtain medical treatment on my behalf if I am unable to do so. I understand that I am responsible for any medical expenses incurred.

Photo, Video, and Media Release
I grant Warrior Moms Support Foundation, Inc. permission to photograph, videotape, and record me during retreat activities and to use such images, recordings, testimonials, or statements for educational, promotional, fundraising, website, social media, and other nonprofit purposes without compensation.

Confidentiality and Respect
I understand that retreat participants may share personal experiences and stories. I agree to respect the privacy and confidentiality of fellow participants and not disclose personal information shared during retreat activities without permission.

Acknowledgment
I have read and understand this Release and Waiver. I sign it voluntarily and understand that it affects my legal rights.
